Before you write
The app won't load. The app runs on Atlassian's platform, so check status.atlassian.com first.
I can't see "Request approval" in the page menu. You need permission to edit the page. Blog posts, whiteboards and databases aren't supported; only pages.
An approver didn't get a notification. The app notifies through a footer comment that @mentions the approver. Check that notification comments aren't switched off in the space or site settings, that the approver can view the page, and that their Confluence notification settings allow mentions.
"This page changed since you opened it". The page was edited while the panel was open. Reload the page and decide on the current version.
The badge still says Approved after an edit. The badge updates when the page loads. Reload the page. If it still shows the old state, the edit event may not have reached the app yet; it will catch up on the next view.
An approver has left the company and the review is stuck. It shouldn't be: deactivated accounts are shown as Unavailable and don't block. If it is, the requester or a space admin can replace the approver from the panel.
Approve and Request changes are greyed out. The app's licence may be inactive; ask a Confluence admin to check it. Statuses and history stay readable.
